By sending this form you agree to pay pre-reservation fee of:

 £ 250 (or equivalent in any other currency) if you wish to be on the top of the list.

The fee is not an obligation, you can be on our waiting list even without it. Making this payment guarantee you preference and being always on the top of the list and receive from us all the documents about the parents as pedigree, health tests, trial and show results etc.

After You submit the form, You will receive to Your e-mail link to pay via card. If you are in United Kingdom, you can also pay by bank transfer. The Sort Code and Account number will be in the same email. You have to pay within’ 2 working days maximum. If we do not receive the payment maximum within’ 2 working days, Your application will be accepted, but with no preferences. You can always contact us and ask, if we have puppies available also.

 

We only accept payment via Wire transfer or by credit/debit card. We do not accept cash, cheques or any crypto currency. If You change Your mind, let us know and we will erase all your data.

We will not transfer the personal data to others except for the specific classes of data transferees as below:

All personal data collected and held will be kept confidential, but where disclosure is necessary to comply with any statutory or legal obligations or requirements or provide the Services to or carry out the original purpose or a directly related purpose specified above, for which the personal data were collected, those data may be provided to the following parties: competent court of law, law enforcement agencies, Office of the Communications Authority or other governmental, statutory or regulatory authorities, institutions or organizations and Kennel clubs.

 

I hereby agree that Lischnitz De Boheme will collect personal data given by this agreement to enable to provide Services to me or to deal with me about purchase and sale of dog. I understand that I may decline to provide the requested personal data, but in such case Lischnitz De Boheme may not be able to provide the Services to me or to sell me any dog. I also understand that Lischnitz De Boheme is unable to process my inquiry if I do not provide the request informations. Personal data collected from me may be used by Lischnitz De Boheme for following up and processing of my application for the Services or dog purchase, enabling the provision of the Services to me, processing of payment instructions or collection of amounts outstanding from me in relation to the provision of the Services or dog purchase, marketing of the products and Services, designing Services for me, following up and handling my complaints and enquiries, making such disclosures as required by applicable laws, rules and regulations, any other purposes as may be separately notified to or agreed with me, including the purposes stated in any special terms and conditions, other terms and conditions and application for the supply of specific goods and services and any other purposes directly related to the purpose for which the personal data were originally collected.
